LOCAL 880 FLU SHOT PROGRAM 2012

Local 880 Annual Flu Shot Program is a great example of how our Union’s Health & Welfare Fund can improve our quality of health and reduce health care costs.  Getting a flu shot can prevent serious illness, which can be life-threatening for the older members of our families and those with chronic illnesses.  As it has in the past, Local880 is again making this program available, which will allow eligible members to get a flu shot from a provider of their choice.  After you submit a receipt to the Fund, you will be reimbursed up to a maximum of $30.00.

WHO IS ELIGIBLE TO GET REIMBURSED FOR A FLU SHOT?

1.  Active employees for whom a Health & Welfare contribution has been made for the month
     in 
which the flu shot is given, and their covered spouses.

2.  Retired members with retiree healthcare, and covered spouses.

PLEASE NOTE THAT THE FOLLOWING INDIVIDUALS
ARE NOT ELIGIBLE FOR REIMBURSEMENT:

1. Medicare eligible individuals, as flu shot providers are required to bill Medicare.

2. Kaiser members, as Kaiser provides flu shots to their members.

3. Employees for whom no contribution has been received in the month in which the
    flu shot is given.
